4 days since appeared. Located top of shaft at base of gland. No diabetes. No sex outside of wife of 15 yrs This has showed before thought it was a bite because there were 4 very tiny marks. And the area had a slight swelling. My body has felt fine no temp. Area does not itch.

Thanks for fresh photo.
It shows the characters of a condition called balanoposthitis.
Also noticed that there is desquamation of the skin of your palms, indicates less integrity of skin and need for proper supplements for the skin like multivitamins, minerals, trace elements.
Well,l the main condition is called balanoposthitis and needs the following management:
Clean the area with plain water 3-4 times in a day, make it dry with cotton cloth or hair dryer.
Apply Liquid Povidone Iodine twice daily.
Use loose underpants and pants to keep the area ventilated.
Avoid hot and humid atmosphere.
In few days this may heal up.
If there is no full response by these means you may need a course of an antibiotic, metronidazole and probiotics; needs a valid prescription.
If your wife also has some problems like leucorrhoea/vaginal discomfort/discharge, it is better to get examined by a Gynecologist and treated if required, I would advise the same course for both of you.
No sex till recovered well.
Protected sex to avoid cross-infection.
Get Blood sugar of both of you done. This may be the first indication of diabetes in some patients.
