Detailed Answer:
Hello and welcome to 'Ask A Doctor' service.
Yes I can understand your concern
ESR and CRP are biomarkers for inflammation. If they are elevated especially in bronchiectasis it means that a current superinfection is already in the airways of that patients
However the more exact for the acute inflammation we use CRP and Procalcitonin. They are more reliable in bacterial infection.
Nevertheless, it depends on the level of elevation and the chronicity of the infection of the bronchiectasis. If it as widespread bilaterally cystic bronchiectasis with levels of secretions and the above blood tests are elevated than we usually start antibiotics treatment.
We prefer the examination of the bronchial secretions to find the exact cause and after the correct treatment.
It would be better if I see the chest CT of the patient and I will explain to you exactly what situation is in the patient.
Hope to have been helpful. Let me know if I can assist you further.

Dr Jolanda

Hi again
I've gone through the chest CT images. This is a cystic bilaterally bronchiectasis. However, there are not so many secretions in it.
Now you can do nebulizations with steroids combined with Albuterol twice for up to two weeks
You should also use mucolytics such as N-acetylcysteine once orally frequently and try to drink a lot of water and do respiratory physiotherapy.

I only have Budesonide 500mcg/ml
And salbutamol 100 mcg and sodium chloride.
Can I mixed them all 3 together?

Detailed Answer:
Hi again
The only indication for surgery in bronchiectasis is massive hemoptysis.
If you say that you have massive hemoptysis several times then I recommend you too to see a thoracic surgeon again.
Hemoptysis in such conditions is not cured with antibiotics, unfortunately.
Please follow your doctor recommendations.
