Question: why would there be a very bad rash on top of his one foot thats swollen

I read your query and here is my advice.

Taking count that he is having swollen feet, ankles and legs for three months he may be having stasis dermatitis or inflammation of the skin because of poor circulation.
Increased pressure in veins damages the capillaries making blood cells to leak in the tissues forming clots and rash.
You can attach a photo of the rash in reports in order that I can evaluate it.
Managing the edema and the blood pressure will improve the rash also.
Using diuretics and keeping limbs elevated may be needed.
