Abnormal Stress Test(Positive For Reversible Myocardial Ischemia. Is Angiography Needed?
ABNORMAL STRESS TEST(POSITIVE FOR REVERSIBLE MYOCARDIAL ISCHEMIA --- DO I REALLY NEED TO GET AN ANGIOGRAPHY DONE? I am 61 yrs old (ex-smoker for past 5-6yrs). I have a stent (implanted in 1996) in RCA. Another stent was implanted in 2011 in LAD. During routine CCC a TMT finding is " exercised for 9.31 minutes on Bruce protocol at a worlkload of 10.9METS and achieved 96% of max predicted HR. Exercise was terminated due to THR achieved. Resting ECG revealed rSR pattern. Developed Asymptomatic 1mm USST dep. in leads II, III aVF during stage 2 at HR of 120BPM which progressed to 1.5-2mm USST dep in leads II, III aVF during stage 4 at HR of 153BPM. ST_T changes reverted to basal pattern within 5.0 mins in recovery period. No significant arrythmia were observed during stress test. Normal HR and BP response to the exercise. No S3/S4 heard. ABNORMAL STRESS TEST(POSITIVE FOR REVERSIBLE MYOCARDIAL ISCHEMIA."
YES – Angiography is needed / that too as soon as possible. Block in the stent and/or native vessel is extremely likely and may need intervention again / Absence of pain is no proof of absence of the problem / So too the effort tolerance.
CT angio (for anatomy) and thallium stress (for Physiology) may give clue and confirm... / But angiography is the gold standard / and you will need it anyway, if any of the too turn out to be positive..
