Are A Possible Subtle Fracture And Sprain Showing Capsular Thickening About The PIP Joint Of A Finger Serious Concerns?
IMPRESSION: 1. The insertion of the terminal tendon (distal extensor tendon) of the fourth finger (ring finger) at the dorsal aspect base of distal phalanx is not definitely visualized suggesting possible tear or avulsion. The central slip of the tendon is seen to insert at the dorsal aspect base of middle phalanx. Clinical correlation for extensor tendon injury is suggested. Flexor tendons are intact. 2. There is bone marrow edema involving the base and proximal shaft of the middle phalanx of the fourth finger. In the setting of trauma this is consistent with a bone bruise. Although a definite fracture line is not visualized, a subtle fracture line or avulsion fracture could be obscured by the bone marrow edema. Correlation with x-rays recommended to assess for a subtle fracture. 3. Capsular thickening about the PIP joint of the fourth finger suggesting a sprain. No definite tear of the collateral ligaments. No definite tear of the volar plate. There is surrounding soft tissue edema greater dorsally.
