Bad Dandruff, Itchy Scalp, Yeast Infection, Fungal. Used Steroid Topical Treatments. What Is Going On?
I have had a very itchy scalp for about 20 years now along with really bad dandruff. It seems to get worse around the change in seasons, but this spring it's particularly bad. It itches unbareably.
I have had some steroid topical treatments prescibed by doctors over the years that did nothing. I have tried every shampoo known to man. I am also a woman and it seems like my hair is thinning on top a little and I'm afraid it has to do with this condition.
I've been reading online that it could be a yeast fungus and perhaps has something to do with my endocrine system or hormones being off. I have had some issues hormonally, but never thought about its relation to my scalp, since I've been living with this for so long it's just become the way it is.
I have out monistate cream on my head and it seems to feel better the next day, but for obvious reasons a cream on my head with long hair doesn't work the best.

From your description it could be either seborrhoeic dermatitis or scalp psoriasis. The chance of it being former is higher. Scalp psoriasis usually has well defined scaly patches over the scalp. Seborrhoeic dermatitis has greasy scales , a severe form of dandruff. The diagnosis can be confirmed by a dermatology consultation.
Seborrhoeic dermatitis is a chronic condition, but very much controllable. It is caused by an yeast called malasezzia. This yeast is a normal resident of human body and once its colony increases you mount reaction against it causing dandruff. This yeast loves oil and therefore it is seen more in oily skinned. That is where hormones work. Androgens increase the production of oil from our sebaceous glands. Worsening of female pattern baldness is described in dandruff but dandruff per se won't cause hair fall.
So the aim of any treatment is to bring down the colony of malasezzia. Anti-dandruff shampoo containing ketoconazole and ZPTO can be used 3 days a week for a month and maintain it atleast once a week to prevent yeast from multiplying. A course of oral antifungals like ketoconazole will help but you need a prescription for it. Avoid applying oil on the scalp.
