Blistery Itchy Spots In Nappy Area. What Is Causing This ?

My 15 month old has suddenly developed clusters of small itchy blistery spots in her nappy area nappy cream is being applied regularly and isnt working and it appears to be spreading but still contained within the nappy area, what could it be and how can i make her more comfortable?
