Hi,thanks for query.Swelling in the scrotum along with heaviness in scrotum are the symptoms of hydrocele.You can not feel your testis at times due to swelling.The diagnosis is just by seeing it.Yes you need to go for surgery even if it is not causing any problems.Ultrasound is done to confirm the diagnosis.bye.
