The acivir will be effective if it is given within 24 hours of the onset of the lesions. I would suggest you to consult a oral physician and get it evaluated. Since you have not described about the lesion, I cant say about the diagnosis, but the herpes infection will not stay for a month, usually it heals by one week. So consult a oral physician and get it ruled out, it can be any vesiculobullous lesion like pemphigus, lichen planus...
