Can Therapy Help To Repair Meniscal Tear Or Surgery Is Only Solution?

I have 2 meniscal tears: one is a complex tear of the posterior body and horn of the medial meniscus with dominant horizontal componet but no displaced meniscal tissue is identified. There is alos a horizontal tear through the body of the lateral meniscus, best visulaized on the coronal sequences and increased signal at the posterior root attachment of the lateral meniscus,possibly related to tearing or fraying. I have disffuse cartilage heterogeneity along the mesial aspect of the weightbearing portion of the medial femoral condyle without a full thickness effect Also a focal near full thickness defect along the posterir weightbearingportion of the lateral femoral condyle series 9, image 20 that mesaures approx 4 mm. I also have heterogeneity and poartial thickness cartilage loss along the posterior aspect of the lateral tibial plateau and mild compartment osteophytosis. I also hae a focal area of full thickness cartilage loss along the lateral patellar facet with underlying subchyondral reactive change and diffuse thinning of the medial patellar facet cartilate with a focal area of full thickness cartilae loss and minimal underlying subcohondral reactive change along with cartilagte heterogeneity with a focal area of full thickness cartilage loss along the trochelar groove. Can therapy help me to avoid surgery or am I a real mess in my knee at this time. Thank you

After going through your query I concluded that you have degenerative arthritis of knee joint. Treatment of your joint depends mainly on symptoms specially pain. First nonoperative trial should be given.It is in form of Diacerine,glucosamine, vitamin D3 if it is deficient and analgesic such as ibuprofen for pain relief. quadriceps exercises and Physiotherpy also done simultaneously. If it doesn't improve in 3-5 months of treatment then it is okay. Otherwise total knee replacement is the only option for pain free joints. You can discuss with your doctor about it.
