As always, it is difficult to provide a concrete diagnosis without examining a patient, or some test results, but here goes....A recurrence of a fissure should definitely be ruled out. Rectal ulcers cause similar symptoms, as do anal fistulas. In the meantime (while you wait to meet your surgeon), it will be a good idea to change ur diet a bit to ensure soft, easily passed stools so as to prevent further trauma and ease the pain. Take more fluids, fibres and soft foods.
