Child Has Fever, Trouble Urinating. Taken Oflomac, Paracetamol. Blood Report Positive For Typhoid. Right Treatment?

Hi, My daughter (4 yrs old) had a fever consistently ranging between 102-104 for the last 3-4 days. I took her to the doctor and he suspected this to be UTI since she also had trouble urinating with frequent urges and irritation....

he put her on oflomac and paracetamol for 2 days and informed us that we'll get a blood report if the situation worsens.

My daughter still didnot improve and had fevers consistent (it comes down for 4-5 hrs after paracetomol but still consistent at 101-102).. we met the doc and he suggested blood and urine tests..

Blood report indicated neutrophilis at 71% and crp at 81.2 mgs/L... urine routine shows traces of albumin with 15-20 hpf pus cell. Blood report was positive for typhoid o and H , but doctor suggested that it might be a false positive correlating with other blood routines...

My daughter is also vaccinated for typhoid.
while waiting on urine culture report, the doctor put her on mikacin inj and cefixime oral (50/5mg) antibiotic..

Today the urine culture came out (after 2 days of mikacin and cefixme) and it says "no growth seen in culture" -

Is this still indicative of UTI or something else..... doc informed that this might be because of the 3-4 doses of ofloxacin that was given prior to taking the urine sample....

I'm concerned and want to find out if she is on the right treatment track.... please help....

Hi Dear
welcome to the board of Health care Magic,
This is sure that your child is suffering with UTI. exact antibiotic to be used can be confirmed by the urine culture only .
Initially empirically antibiotics are started so that to stop the infection flare up, till the culture report is available from the lab. usually it takes 72 hours to get the culture report,
In the mean time we should also go for blood culture to confirm the typhoid fever.
Apart from the widal test ( which at times is false positive) we can think of Typhi dot test (IgM&IgG detection) , which is more informative and accurate than the widal test.
If theere is positive report of typhoid only thenwe should think of treating typhoid, other wise your child should respond to the treatment of UTI only
