My 5 year old son complains about it hurting when he urinates, he also has complained of his back hurting occasionally. He has never had accidents in the past put has had episodes now. When I look at his underwear he usually has a wet spot in the front as if he is leaking urine . I have brought him to the doctor for U/A and they come back clear. I am worried and don t know what step I should take next. Please help!

Hi,
Complains of painful urination in a child is usually due to Urinary tract infection. Urine analysis may not be of complete diagnostic value, if the above complain is associated with fever, burning sensation he needs to be treated for UTI.

Male children are commonly known to have vesicoureteric reflux disease, which can produce symptoms like urinary tract infections. This also cause urinary incontinence ie urine leakage. Get him to a pediatric surgeon or nephrologist for a Ultrasound scan and some other test for identifying the problem.
Bye.
