Constant Pain In Body, Out Of Control Glucose Level. History Of Having Diabetes Since Childhood. Any Medication?

Im 47 with type 1 diabetes. I have all my limbs and I'm blessed to still have my sight. I have had diabetes since about age 3. I'm just having a very hard time with it right now. I'm in pain, you might say all the time. It's even more difficult to exercise. Merely walking can be very painful. I used to work out everyday. I guess I just wanted to share. Or should I say, Is there anything on the market? Oh, my glucose levels are just out of control. Thank you!

General & Family Physician 's  Response
Hello,
If somebody is having a uncontrolled diabetes whether type 1or2,the most likely cause for any diabetic having pain in leg or foot is due to diabetic peripheral neuropathy although it could be caused by deficiency of VIT B 12 or osteoarthritis.
Although much studies and understanding has been reached regarding the patio physiological process leading to such type of pains, not much headway has been achieved regarding it's treatment.
Treatment is aimed at very tight control of blood sugar levels and medications to treat the pain.
There are various types of medications like antidepressants and anticonvulsants have been tried but FDA has specially approved three medications for treatment of neuropathic pains. These are antidepressant DULOXETINE,anti convulsant PREGABLIN and long acting opioid TAPENTADOL ER.
You may also take injectable VIT B12 and VIT D supplements,as these may help you a lot.
Discuss about these with your treating doctor.
Thanks

Diabetologist Dr. Pratap VGM's  Response
Dear sir/madam

Long duration of diabetes with uncontrolled hyperglycemia is associated with hyperglycemia neuropathy which involves the long nerves.

The best way to treat it is by
1. Good control of blood sugars with basal and bolus insulin therapy
2. Methylcobalamine + pregabalin combination of tablets if there is no contraindications.

Thanks
