Constipation, Bloated Feeling Under Diaphragm, Pressure, Diagnosed Uterine Fibroid. Treatment?

I am a 72 y.o. white female and have been experiencing severe constipation for several years. Now, however, after the 4th day of no bowel movements, I have a high bloated feeling under my diaphgram and down the right side . It There is no pain, but lots of pressure. I also have uterine fibroids Dx by my Gyn. They have never given me any problems but could thiis pressure be from them and/or the constiipation? I have no appendis. Is this an emergency? DO I need to go to the emergency room or would follows ups with a GI and GYN in the next few weeks be appropriate. Thank you. Kit Blaker

Hi ! Good evening. I am Dr Shareef, a general surgeon, answering your query.

From your history, it appears that you had an appendix surgery in the past.Sometimes there are adhesion formation between the internal post operative scar and the intestines, and if it is extensive, the bowels will not be able to move, and there will not be passage of stool, and gradually the gases also. This is known as intestinal obstruction which may be acute requiring urgent action, or may be subacute troubling you for a few days, and getting relieved of its own later. Basing on this, if you are just constipated and are passing flatus, then you may do the follow ups with your treating doctor. However , in case of absolute constipation (no passage of air or stool) followed by increasing distension, you may have to report to the emergency room to get examined by the doctor in person for further investigation and management.

The fibroids in your uterus has to grow to that extent to exert pressure on the intestines to cause distension and so this may not be considered to be the cause for it.
