Your mechanism of injury seems relatively mild to cause bloody coughing. Theoretically, a hard blow to the back could cause a lung injury or even a broken rib but you haven't described that type of pain. However, if you are coughing up blood, this should be evaluated. Call your primary health care provider or consult the ER.
