Could The Impingement Be The Cause For Lumps On The Fingers?

I have an impingement in my shoulder - it's about 2 years old and well managed with cortosone. I've lost 100+ lbs and had excess skin removed from arms 6 months ago. I have well controlled diabetes (6.1 A1c). Starting 3 weeks ago my hand started swelling and hurting. I can tell my arms are also swollen but not as bad. I've had pblms with the hand and had a cortosone shot there as well. There are big knots at the end of my fingers, I forget what they are called but and Ortho diagnosed it. I think either the impingement is the problem or possibly lymphadema - but there's no lump(s). I'm away from home for 2 months. Is this bad enough to seek medical help ASAP? I cannot close the hand, but if I raise it for hours above my heart it seems to help. As a diabetic, my major concern is chopping off my bits - I'll head to the nearest urgent care if necessary.
