Could The Visibility Of Cystic Lesion In Abdomen Be Due To Ectopic Pregnancy?

Good Afternoon! Just found out I was pregnant. went to the ER because I was having lower left quad pains. I have a history of endometriosis and polyps (last lap was 8/8/13). Blood and Urine tests both came back positive. HCG levels are 4160.00 I was told that they can not see a baby as of yet and that the only thing in view was a cystic lesion (my guess scarred tissue from surgeries). Please advise as I m very nervous! Does this mean that this could be an ectopic pregnancy or is it too early to tell?

Good Afternoon.

I am Dr Ramadevi Wani. I will be answering your concerns today.
Did you have a transvaginal ( internal) or transabdominal ultrasound? With HCG level of 2000 pregnancy sac in the uterus is seen by transvaginal ultrasound almost always. By Transabdominal ultrasound, pregnancy sac in uterus is seen with levels of HCG above 5000. So if you have had a transabdominal ultrasound, it is normal to not see a pregnancy sac.

If you had transvaginal ultrasound then the possibilities are:
1. Ectopic pregnancy.
2. Failing pregnancy- Pregnancy which is not growing normally.
3. Normal pregnancy- Very rarely a transvaginal ultrasound may not show pregnancy sac.

The cystic lesion could be ectopic pregnancy. It could also be a normal corpus luteal cyst ( pregnancy cyst).
To clarify things, repeat the HCG levels after 48 hours. If the increase is more than 66% it suggests normal healthy pregnancy. Repeat the ultrasound again. By then you should be able to see pregnancy sac in the uterus.
In the mean time if the abdominal pain worsens, you must go to ER as it suggests ectopic pregnancy
