Diabetes, Black Color Shoe Bite On Foot. Soframycin Prescribed. Is This Ointment Safe?

Dear Sir,

My father is a diabetic patient , his sugar levels are Fasting 94 , PP 165 with insulin .
from the last 7 days we are seeing shoe bite in his foot , the day we noticed the same , We immediately went to the skin specialist doctor at TMH ( TATA MAIN HOSPITAL ) ,Jamshedpur , they sent us to the surgical department , surgical doctor there had given an ointment named “ soframycin skin cream “ . the shoe bite is of black color , in the side of the second toe , and there is no pain in the shoe bite .

Please advice to cure the same whether we need to apply this ointment on a regular basis as advised to us by the doctor of TMH or we should show in some other hospital in the surgical department . Please recommend any good hospitals in India for this case at the earliest as we are in tension .

Hi,
Welcome to HCM,
Your father is diabetic and taking treatment with reasonably controlled blood sugar, as he is taking insulin please continue to take insulin as directed by Physician.
He has developed ulcer with black toe which could be due to shoe as diabetic patients develop peripheral Neuropathy (Nerve damage) hence pain is not there.
If this black colour please also take care that it is not gangrene because if blood supply to toe is reduced then this can occur.
This is good that you have consulted surgeon he is right doctor to be consulted and follow his instructions.
Please also get his complete check up with internal medicine specialist /Endocrinologist to examine and find out if he has neuropathy and other complications in heart or kidney, by Colour Doppler of lower limbs, nerve conduction study and ECG and echocardiography.

Hello,
Long standing diabetics with chronically elevated sugar levels cause diabetic sensory neuropathy and peripheral vascular disease.
When these two get combined along with an infection,it results in diabetic foot ulcer which when untreated leads to gangrene.
I am sorry to say that your father is having diabetic foot ulcer with gangrene.
This is a real dreaded problem and requires immediate attention by a team of specialist doctors.
It requires a multi disciplinary approach.
You must get him admitted in a super speciality hospital.
Treatment involves...
Wound culture for proper antibiotic therapy.
Investigation and treatment of vascular insufficiency ,which is done by a vascular surgeon who can either do angioplasty or stenting or some bypass.
The degree of peripheral disease will decide whether a local debridement is enough or some vascular reconstruction and / or amputation is required.
Don't waste time and do it fast.

Hello,

Your father is a diabetic and his blood sugars are controlled quite well with insulin.
Now he has a blackish patch over his foot.
Diabetics are prone for diabetic foot ulcers due to reasons like Diabetic peripheral neuropathy and uncontrolled blood sugars.
If not identified early and treated effectively the complications which may follow are diabetic foot ulcer superadded with infections, gangrene.
Black often represents gangrene.
Second opinion may be needed.

Hello,
Thanks for the query to H.C.M. Forum.
Diabetic foot ( ulcer over foot due to diabetes).
Three type of diabetic foot is there as,
1. Neuroischemic foot ,in this occlusive vascular disease is the main reason ,pain at rest,ulceration at margin of wound ,necrosis,gangrene.
2. Neuropathic foot, in this type usually there is no pain but ulceration, this type of wound is warm,dry, painless and pulsation is there.
3 Infected foot ,most common problem in India,usually black due to necrosis.
Your 's father wound seems to be this type of wound.
Now your query is how to treat without surgery.
Yes, this is possible, consult a diabetologist and sterile dressing after cleansing with povidone and high antibiotics ( gram --ve,gram +ve, anerobes) is advised.
Control of diabetes is must.
Healing of wound will take some more time as an ordinary wound because of diabetes.
Don't be panic your father's would be alright.
