Do Persistent Cough And Swollen Ankles And Feet In A 93-year-old Require Immediate Medical Attention?

I am inquiring about my 93 year Dad who has refused to see a doctor. He has had a persistent cough for about 4 weeks, approximately 2 weeks ago complete loss of appetite. He has swollen, bluish ankles & feet. He has always been in excellent health & takes no medications. I realize at 93 your overall health is naturally declining. Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Symptom of cough more than 4 weeks needs to be brought for investigation because it could be pulmonary pathologies like mycobacterium infection, bronchitis (Allergic condition ) bacterial infection like pneumonia, and malignancy, cough symptom of more than 4 week can not be taken as granted, routine blood tests, imaging studies (X-ray, CT MRI) advisable.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Akhtar Husain
