Hello, and I hope I can help you today. I am assuming because you are on
Glyburide that you are not a juvenile diabetic or someone normally with brittle control. You are unlikely to go into DKA if you are an adult onset or gestational diabetic, so going to the ER is not likely necessary.
First, please recheck your blood sugar first. If it is under 300, it is OK to remain at home and take your medicine again as prescribed and retest in an hour or so, but I do agree you need better control of your sugar. You may need an increase of your glyburide or the addition of
insulin. You should continue to check your sugar fasting, and before and either 1 or two hours after every meal. Yes- it is necessary to check around 6 times daily to have enough information for your doctor to adjust your medicine properly. Also please write down what you eat so they can also determine if this high sugar is because of your diet or another reason.
I would contact your OB provider as soon as their office opens so that they can adjust your medication
dosing or switch you to insulin if they think it is necessary. You should be seen at least weekly by your OB doctor to adjust your medication and monitor the baby.
Having your sugar out of control can increase your baby's risk of getting very large as well as an increased risk of stillborn so it is very important to work very hard, especially in the last month to help prevent this from happening again.
I hope my answer was helpful to you and good luck with the rest of your
pregnancy,
Sincerely, Dr. Brown
