enlarged cardiac silouette- means that the heart is bigger than it is supposed to be.
vascular congestion of lungs means that there is some sort of back pressure from the left heart to the lungs, due to which blood in the lung vessels has increased.
These two features is suggestive of a congenital heart disease in a 20month old baby.

enlarged cardiac silhoutte means the patient has cardiomegaly. With congestion means an obstruction or impairement in the flow of blood. It could be due to a atrial or ventricular septal defect. I would advise you to get a 2d echo of the heart and consult a cardiologist in this regard. I hope i have answered your query. Take care
