Experiencing Pain Around The Shoulder Due To A Sling

I hurt my left shoulder a few weeks ago. I wore a sling to help allieviate the stress on it. Now it constantly aches and wakes me up at night with the pain. When I move my shoulder its almost like a grinding feeling all the way down to my shoulder blade and it makes a crunching sound. Should I be concerned?
