hi welcome to healthcaremagic

Varicocoele is abnormal dilatation of the veins in the testes , this results in pain.... during standing, playing ,any kind of exercise there will be flow of blood towards gravity hence collection of venous blood in panpiniform plexus(vein in testes) causes pain... surgery is the best treatment available... you can consult a surgeon..
