Hi friend,
Welcome to Health Care Magic

This level of blood pressure could not account for the symptoms...nor the size of the graft..
You have not stated your personal habits, family history, or co-morbidities, even age...which could have an influence on the diagnosis and outcome...
She needs investigations for heart failure and other causes – X-ray chest / ECHOcardiogram to assess ejection fraction / Lab tests for kidney function and electrolytes / and so on, depending on clinical assessment.

See your doctor..

Take care
Wishing speedy recovery
God bless
Good luck
