Hi,
Welcome to HCM.

Many things mentioned by are not able to understand.
I understand your age is 50? You are having HIV through blood transfusion since 9 years. It means that you got HIV 41 years back. It could not be possible as HIV was known to Humankind for the 33 years only. Even if it was traced back from the pre-AIDS era, You are the longest known survivor among HIV patient that too with this viral load. Kindly clarify the same.
Regarding your viral load, it is very high only. Are you not on ART? If you are on ART, it is a high time to have a viral resistance test for ART Drugs and according to the sensitivity pattern, the combination of drugs for you have to be selected from NRTI,NNRTI, PI, Integrase inhibitors, or entry inhibitor groups of drugs which would not give much trouble for your Interhepatica colestasis problem.
