my uncle is 70+ years old, he is having chronic renal failure + congestive heart failure+ hypertension+ hyperglycemia and chronic severe anemia . His Hgb is less than 7.0, we always to infuse him with 4-5 pints of blood every month. I have heard about EPO, that I can reduce his dependability on blood transfusion . if so what dose you do recommend plus what type of antihypertensive group or combination of groups he must be using for long term. also what would be standard medications for such patient as I don t think his doctor is treating him good.

EPO is erythropoietin used in cases of cases of chronic renal failure,it is quite helpful in reducing the transfusion burden and it has to be taken on a regular basis. Doses for it is 50-100 units per kg.
In chronic renal failure few antihypertensives like ace inhibitors can not be used if the creatinine is very high, generally calcium channel blockers like amlodipine and diuretics are the first choice drugs.
Also your uncle needs aspirin , antidiabetic medications and statins for the cardivascular risk reduction.
Calcium supplementation is rule in CRF patients and in some cases other drugs like pottasium binders can be given on case to case basis.
