Have Pericoronitis, Wisdon Tooth Extracted. Swollen Jaw, Painful. Suggestion?
I have pericoronitis and had cause of antibiotics but my jaw is swollen and very painful. The antibiotics does not seem to work and the pain killers not strong enough. I feel the infection has spread to the mouth. I am going to have the wisdom tooth extracted but I cannot do anything until the infection has gone please help in much pain

Pain and swelling can be because of acute infection in the gums and soft tissue which are surrounding the wisdom tooth. In case of partially erupted wisdom tooth, flap of gum tissue is created where food impaction can occur. This condition is known as Pericoronitis and in this condition, gum tissue will appear swollen and gum tissue will appear red. Pus discharge may occur from the gums. There is difficulty in swallowing; closing the jaws and bad taste and odor is present from oral cavity. For treatment of Pericoronitis, debridement of area or operculectomy that is surgical removal of flap over partially erupted wisdom tooth can be done and if the condition recurs, then extraction of wisdom tooth may also be required. You can visit your dentist for evaluation and treatment. By the time, you visit your dentist, you can do warm saline rinses 3-4 times a day to reduce the swelling of gums and keep wisdom tooth area clean. Food accumulates in relation to wisdom tooth will aggravate the pain.

the type of history you are presenting here directly indicate to your wisdom tooth infection.
In this situation pain may refer to ear, neck & head.
immediately start up with warm saline rinses & betadine rinses, along with antibiotic course including amoxycilline 500mg TDS & ibuprofen 400 mg TDS.
Than visit to your dentist for check iopa x-ray and removal of this tooth.

Pericoronitis is the inflammation of the mucous membrane overlying the wisdom tooth .
this area is susceptible to frequent infection as the place is difficult to clean and the gum pocket incorporates food debris , plaque and other irritants leading to infection and abscess formation.
i suggest you to maintain good oral hygiene by tooth brushing and using the antiseptic mouthwash like chlorhexidine for oral rinsing that prevents infection.
you can start with Antibiotic Augmentin 625mg with analgesics twice daily for 3 to 5 days which will subside infection and swelling.
visit a dentist , get a x ray done for your tooth and if its impacted go for surgical removal of the wisdom tooth whcih in turn will remove the gum flap as well.
temporarily OPERCULECTOMY , that is excision of pericoronal flap will give you relief .

In case of pain and swelling associated with wisdom teeth, you are right about pericoronitis. Antibiotics are the first line of treatment. You must take novaclav 625mg twice daily along with vizylac capsule. For the pain you can take ketorol dt - 2 tabs stat ( dispersed in water). It gives instant symptomatic relief. Also, scaling around the infected 3rd molar and irrigation with a mix of hydrogen peroxide and saline can give much relief as it shall remove all the debris accumulating around the tooth and the pericoronal flap.
Warm saline rinses also help in such impaction cases.
Opuculectomy, i.e. surgical excision of the pericoronal flap around the tooth can be done before you are ready for the final extraction.

as you know you have pericoronities,your dentist must have given you antibiotics along with analgesics.
if you are not finding them suitable you can get them changed but please take the complete course of medication else it would not help you much.
this will certainly reduce your pain and infection.
simultaneous do warm saline rinses which will localize your infection.
there are morphine based pain killers too.you can take them after consulting your dentist.
remember antibiotics and analgesics are temporary solutions in case of dental treatment.you must plan and get your tooth extracted only then it will be cured completely.
meanwhile maintain very good oral hygiene.
you can also do one thing locally
that is you can irrigate your pericoronal flap with antiseptic solution.this will push the debris and food particles out and gives immediate relief.
