Heart Failure, Diagnosed With Future Damage Of Mild To Mid RCA, Mid LAD And Ostial Diagonal Disease. Meaning?

My mom has severe heart failure with an ejection fraction of less than 20 percent, she has been back in hospitalized has been diagnosed with further damage of mild to mid RCA disease mid LAD disease,severe ostial diagnol disease want does this all mean, we have been told the outlook is bleak, but with this further diagnosis what does this mean for her long term prognosis please.

She has block in the blood supply to her heart.
Her heart function is poor
The prognosis does not seem to be good – apart from ejection fraction, it also depends upon valves’ integrity and function / wall of the chambers / arrhythmia, their nature and frequency / renal function / response to medicines.     
You have not stated her personal habits, family history, or co-morbidities, even age…
Further deterioration can be prevented or postponed by aggressive management.
If there is further deterioration, heart transplantation may be the answer...
The treating doctor alone can suggest further based on his assessment of the situation.
