Hi,
From history it seems that you might be having coronary insufficiency.
Physical strain can cause more coronary insufficiency leading to angina pain.
Consult cardiologist and get examined and undergo test like stress test etc.
Ok and take care.

Cardiologist Dr. Anantharamakrishnan's  Response
Hi friend,
Welcome to Health Care Magic

An abnormal ECG is an indication for further evaluation. HOLTER (24 to 48 hour ambulatory monitoring) / ECHOcardiogram / TMT (Treadmill exercise ECG) / LABORATORY work-up (electrolytes) – may all be necessary for further assessment and assistance. It could very well be a normal variation...
Your symptoms do not favour heart disease.../ or lung problem!
Breathing difficulty could be from pain or anxiety / it is better to be seen by a Pulmonologist; have an X-ray and spirometry – to exclude any issue
     Discuss with your GP

Take care
Wishing all well
God bless
Good luck
