How Can A Hammer Toe Be Fused If There Was No Broken Bone?

I went in to get my hammertoe evaluated and the doctor refused to give me any treatment, because I have contact dermatitis on my left foot. When I asked him if you could just cut the ligaments/tendons, he said "no" because my hammertoe was fused? How can a toe be fused if there was no broken bone?

If the joint cartilage gets damaged,then over a long period of time the bone ends get fused/arthodesis.
