Hi,

As per your clinical history, please follow like this:

1) Apply OTC [Nitrogylcerin] ointment in the anal canal at bedtime. Use [lidocaine cream] before 15 mins to the toilet. After coming from the toilet, use a cream containing [Beclomethasone Dipropionate + Phenylephrine hydrochloride].

2) Do sit in warm water[sitz bath] up to 20 mins three times a day to improve blood circulations to anal muscles.

Do follow lifestyle modifications like this to prevent constipation:

1) Try to increase your fiber intake. Adding fiber to your diet increases the weight of your stool and speeds its passage through your intestines. Slowly begin to eat more fresh fruits and vegetables each day. Choose whole-grain breads and cereals.

2) Do exercise most days of the week. Physical activity increases muscle activity in your intestines. Try to fit in exercise most days of the week. Don't ignore the urge to have a bowel movement. Take your time in the bathroom, allowing yourself enough time to have a bowel movement without distractions and without feeling rushed.

3) Do take a stool softeners regularly at bedtime. It moisten the stool by drawing water from the intestines which include Colace and Surfak and these are available without any prescriptions.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Uday Nath Sahoo
