General Surgeon 's  Response
Hi.
Thanks for your query.
Noted your history of fistula in ano, internal opening in the rectum means this is a high anal fistula.
This is very typical to treat as this is not curable in a single stage.
Hence the following are the options:
A general Surgeon or Proctologist would operate you in stages so the at the internal sphincter is saves and thus the continence.
Secondly a seton can be passed.
Ayurvedic version if Kshar-sutra.
All have its advantage or disadvantages, discuss with your referring Doctor so that you get an appropriate education, information and then can decided on your own which therapy will be suitable for you.
