How Can Lateral Foot Pain With Degenerative Changes Be Treated?

I ve been having lateral R foot pain for over a year inspite of inserts, custom orthotics and expensive shoes. I just had an MRI of my right foot and it showed full thickness chondral defect. I also have degenerative changes in the 2nd, 3rd and 4th metatarsals. I am wondering what causes this and options are for treatment

you may consult an orthopedic doctor specialized in foot diseases for evaluation of complete foot bio-mechanics,anatomy and who shall also evaluate the ankle, knee for studying weight bearing axis of leg and foot,for the arches,for distortion of tarso-metatarsal alignments etc.Then he shall know the cause for degenerative changes and allow for correction accordingly with orthotics necessary/or some corrective surgical osteotomy/medications etcAs of now, take pain meds and avoid stress on foot by less walking,do contrast bath (alternation of dipping foot in cold water and hot water for relaxation of foot muscles and ligaments,arches.
