Hello. Thank you for writing to us at healthcaremagic
Cystic acne is the severest grade of acne and notorious for causing scaring. Treatment of acne should be apt to the severity of acne. For cystic acne the drug that is approved and works best is Oral Retinoids i.e Isotretinoin. Oral retinoids are the most effective drugs for cystic acne. They are antiinflammatory, comedolytic and are sebostatic i.e reduce sebum production. Besides Oral retinoids, I also usually start my patients of nodulocystic acne on Oral antibiotics lime Azithromycin. Topically you may use benzoyl peroxide gel as a spot treatment for active acne. Oral retinoids as well as Oral antibiotics are prescription drugs, I suggest you to visit a dermatologist in your region for a visual assessment of the severity of your acne and appropriate prescription.
Cystic acne is considered to be the most severe type of acne and Oral Retinoids i.e Isotretinoin is the drug of choice in these type of acne. Retinoids bring about the most dramatic improvement in cystic acne on account of the multi-pronged mode of action i.e comedolytic, antiinflammatory, sebostatic (decrease sebum over production) and normalization of abnormal keratinization. Laser's are not meant for active acne. They are mainly for post acne scaring. Besides oral retinoids I would also suggest topical antiacne antibacterials like benzoyl peroxide / clindamycin. Oral antibiotics e.g Azithromycin, are also of benefit to suppress the inflammatory flare that may be associated with the initiation of treatment with Oral Retinoids. I suggest that you visit a dermatologist. Your dermatologist will assess the severity of your acne and advice accordingly.
