How Can Severe Pain In The Shoulder Be Treated?

I have pain in my left shoulder, since i fell down the stairs. To protect myself i grabbed side bar tightly. It gives a jerk to my hand. Afterwards I can not move my hand freely also there is a krack sound and now my full hand sleeps for some time. If turn on my left side while sleeping it hurts a lot. What should I do.? I ve done physiotherapy 10 sitting. But no development.
