ESR is a nonspecific inflammatory marker, and it can be raised in any inflammation or infective etiology or in malignancy condition.

Total WBC count is also high here. So, infective etiology could be a possibility and antibiotic course prescribed if needed. Based on the provided history this much comments can be given.
