She is having Post-Herpetc Neuralgia (PHN) which is pain persisting more than 1 month after herpes Zoster/shingles has healed. This is a common sequelae of Zoster specially in old age group. Individuals can continue to have shooting pains, burning, stinging and stabbing sensations, long after the episode of shingles has healed, lasting months or even years.
PHN is severe and lasts longer in old as compared to young individuals. The pain is due to inflammation and scarring of nerves affected by the episode of shingles.
Treatment is effective but may be required to be continued for months or years, specially in old age group of patients:
Treatment options are:
-NSAIDS
-Tricyclic antidepressants(Amytriptyline,
nortriptyline)
-Gapapentin/Pregabalin
-Carbamazepine
-Topical Capsaicin gel/cream
-Topical
Lidocaine gel/cream
I usually treat my patients with a tricyclic antidepressant,
Amitriptyline once daily at night (the dose can be titrated to twice or thrice daily depending on response) along with
Gabapentin or Pregabalin ( dose is once ,twice or thrice daily depending on response).
An oral Vit B12 Methylcobalamine in doses of 500 mcg to 750 mcg is also useful for neuralgic pains.
As these are prescription medicines, therefore I would advice that you visit a
dermatologist and get them prescribed.
