How Should Scrotum And Underarms Itching With Swelling Be Treated?

I need to consult a dermatologist for treatment of itch in scortum and underarms. Application of 'Itchguard' ointment with Clotrimazole since 2 days has not helped. Certain itchy area of the scortum has become swollen with red trench lines. Pl help.
