How To Bring Down Serum Urea From 88?

My wife is Diabetic taking Insulin LANTUS 16U night at 11am,HUMALOG 20-22-20 Unit with BF,L,D.Her Kidneys are getting effected.Her todays Serum Urea is 88 and Serum Creratinine is 4.2 mg/dl and Hb % is 8.30 g/dl.
What treatment should be taken to bring Urea and Creatinine down. Her Bp is 140/80 today.She was taking Ramipril 5mg in the morning plus Telmisartan+Amlodipine+H for BP,but today it has been changed to Omlisartan 40 at BF and Omnipress xl 5mg after dinner.

I am sorry for the situation you are in.
the medication what your are taking are very good
The raise of urea and creatinine is due to damage in kidney.
this Damage is most of times not reversible.
i suggest you to keep your sugar in good control to prevent further damage.
Try to meet a nephrologist
