I have a patient who has recently developed very painful feet with multiple deep cracks. She is a nurse and is finding it hard to work. I've tried lubrication and cortisone cream but to no avail. She herself has tried antifungal creams also without benefit. I would appreciate any advice and suggestions.
Hello, Thank you for posting on HCM. Te condition you ave described seems to be plantar keratoderma. I would insist you to visit your dermatologist for definitive diagnosis. I would recommend some basic laboratory tests like CBC, Thyroid function test, Blood sugar, etc. Oral corticosteroids and methotrexate can be used but strictly under doctor's prescription.Daily immerse your feet in lukewarm water for 10-15 minutes followed by application of ointment containing halobetasol and salicyclic acid over thick,scaly skin and mupirocin cream over cracks/fissures. After one hour gently apply Xerina cream/lotion all over your soles and lower feet. Take antihistaminics for itching if required. Avoid using tight footwear and use footwear with soft/cushioned sole. Avoid wearing any metal ornaments and contact with detergents/chemicals/dyes etc. Hope this will take care of your issue. Thank you Dr Hardik Pitroda
