Your problem can be helped by wearing tighter underwears or even a sports type innerwear or supporter,which lift up the genitalia.
You must exercise to reduce your body weight.200 lbs with height of 5.4" is overweight. So, exercise regularly.
If you have been married for more than a year, you can get your semen tested after abstinence from sex for two days.
