a small tag on the underside of my tongue is painful when I use my tongue to extract something from between my teeth. Sometimes I can forget about it. Other times it is painful for no evident reason. Will it go away by itself or do I need to be concerned?

It can get inflamed due to physical trauma, burning with hot food or infection.. Other cause of painful skin tag can be a torn piece of mucosa due to injury caused by some hard food or anything else.. I would suggest you to consult an Oral Physician and get evaluated and he can advise anti-inflammatory medicines like Naproxen or Ibuprofen and cold water rinses for relief.. Avoid taking spicy, hot and hard foods to prevent further irritation. You can also apply a numbing gel over the swollen area for relief from pain..
