How To Treat Pancreatitis?

My daughter was told she has a mass the size of a golfball at head of pancreas. The surgeon sais it was porous. Stated it is chronic pancreatitis he said most surgeons would operate but he is going to treat by no eating food through pic line and pain meds. Should they be doing surgery. I am concerned. She is in a lot of pain

In patient who are stable or uncomplicated, the pseudo cysts treatment is supportive. Pseudo cysts that are >5 cm and persist for more than 6 weeks, should be considered drainage.


Detailed Answer:
Hello,
Thank you for using HCM.
There are two possibilities for pain origin in the case of your daughter.
1- She might have a accutisation of the chronic pancreatitis. For sure this is supported by laboratory exams results such as high white blood cells levels, high amylase, high lypase, and high levels of the inflammatory indexes, plus imaging tests result (abdominal ultrasound, CT scan report). Being in a acute phase changes the treatment approach. She should be on analgesics, IV fluids and colloids, IV antibiotics, and no oral alimentation, till she passes this phase. Most (90 %) subside over a period of 3-7 days.

2- The mass in the head of the pancreas, as you are prescribing it (porous), is more likely to be a pancreatic pseudo cyst, which has been developed , probably over 1-4 weeks. This pancreatic pseudo cyst may also cause abdominal pain as the most usual complain, and a tender upper abdominal mass may be present. It can be detected by abdominal ultrasound or CT scan. In patient who are stable or uncomplicated, as it can be in the case of your daughter, treatment is still supportive.
Pseudo cysts that are >5 cm and persist for more than 6 weeks should be considered drainage. In case that pseudo cysts became complicated by hemorrhage , rupture, or abscess, surgery should be performed.
So in both cases the right choice for the moment is to make a supportive treatment, monitoring strictly the symptoms ,the laboratory and imaging exams results.
