In Which Condition Is Clonazepam, Adderall, Prozac, And Xanax Given? Having Severe Interactions

Why would my doctor/nurse prescribe 3 meds that are severe cases that may result in coma and even death ?
The first three are clonazepam, Adderall (amphetamine / dextroamphetamine) and Prozac (fluoxetine)
Sometimes Xanax (alprazolam) This is a very bad interaction and no wonder this website has explain most of the side affects I've had for a long time. Do the Doctors or Nurse look up Interactions FIRST before they prescribe or even the pharmacy ?
