I have gone through your question and understood your concerns. As evident from your question, to start with it was a
Perianal abscess which was drained. Later on another operation ie draining and seton insertion, which is done for Fistula in Ano. Fistula in Ano, formation is common after perianal abscess. It appears that your fistula was inadequately treated that’s why the tract persists, resulting in passage of gas from the anus/
rectum through this fistulous opening.
You need to undergo some investigations like fistulogram and
MRI Perineum to know about the tract of fistula followed by planning for the operation which can be
fistulotomy or fistulectomy. Nowadays endoscopic treatment is also available.
