Ones a woman has sex for the first time, her hymen gets broken and if after that she never has sex again, her vaginal wall will constrict but her hymen can never be reformed. Some women can still experience post-coital bleeding after staying for so long without meeting with a man but it does not mean that they are still virgins. Now to answer your question, when there is no hymen present, it means the woman had had sex before and if there is hymen it means the woman had never had sex. No artificial hymen can be placed to a woman after the first is bridged.
Also, I do not know for what reason you wish to know if a woman had had sex before or not, but personally, I will suggest you should not bother about that as if you want to use that to define your relationship, you might end up feeling ruined or ashamed of her. Just take her for what she is and give her what you can give when you can give.

Your question is about virginity, A virginity test is process of determining whether a female person is a virgin, i.e., whether she has never engaged in sexual intercourse.
Virginity testing is a very controversial and unethical practice.
Virginity testing is not a reliable indicator of a female having actually engaged in sexual intercourse.
Apart from rupture of hymen it can be assessed by laxity of vaginal muscles,but remains questionable.

Some are born without hymen and it may be ruptured due to other causes.
A female can undergo a surgical procedure, hymenorrhaphy or hymenoplasty, to repair or replace a torn hymen.
