Frozen shoulder mostly is of no cause. In your situation probably it is secondary to cervical spondylosis. If there is any nerve compression or disc structural deformity , surgery stands as a mainstay and later followed by physiotherapy.Considering you have no compression , you should go for physical exercise on the first go and surgery later , if results are not comfortable.Even when surgery is done , physical therapy has to be continued. If wearing a neck collar / brace , please do not use it for long hours .That can worsen the symptoms.
Take care.
