As per the history you are 40yrs old (gender not mentioned), with a BMI of 29.4kg/m2 (overweight/obese), on oral antidiabetic drug (glynase-mf) for 6yrs and a fasting glucose level of 150mg%. I would like to enquire about your 2hrs PPBS levels and HbAic.
Glynase-MF contains two fixed drug combinations , glipizide 5mg (
sulfonylurea) and metformin 500 mg (biguanide).
Firstly, Glipizide belongs to a class of drug called Sulfonylurea (SU), which are insulin secretagogues (i.e they increase the Insulin secretion from the pancreas). Since they act by increasing the Insulin secretion, they are proaned to cause
hypoglycemia (
low blood sugar levels), specially in a background of drug-diet imbalance. Hence they should be taken shortly before meals. They can also cause weight gain. Their use in individuals with significant hepatic or
renal dysfunction is not advisable. It is advisable not to take alcohol with these drugs as they may cause interactions. Glipizide is one of the preferred SU.
Now coming to Metformin - this one of the preferred first line agents in the management of Diabetes. They are weight neutral (infact can cause weight reduction) and do not cause hypoglycemia (since they act in a manner different from the drug mentioned above). It can cause symptoms like Diarrhea and nausea. This drug is contraindicated in renal dysfunction (
Serum creatinine more than 1.5mg% in males) and congestive cardiac failure. Metformin is also known to cause
Vitamin B12 deficiency, which can cause neuropathy (it can mimic
diabetic neuropathy). It is advisable to get your viatmin B12 levels checked.
Since your BMI is 29.4 kg/m2, I would advise you to lose weight.
