It is not an uncommon presentation. A heart attack pain has been known to start in the right wrist and gradually climb up to the shoulder and finally settles in the chest and the classical left arm and/or back. This is because of the doubled sided autonomic innervation of the heart developing as a midline structure originally and radiading to the corresponding somatic nerves Hope this answers your question.
